BAPTISMAL INFORMATION FOR THOSE CHILDREN PREPARING
FOR FIRST RECONCILIATION AND FIRST EUCHARIST

It is necessary to have baptismal information on all students who are preparing to receive the sacraments of First Reconciliation and First Eucharist this year.

Parents, please fill in the following information carefully and submit this form by clicking SUBMIT below. In addition - you must drop off/mail your child’s baptismal certificate to Terri Kluesner at the Faith Formation Office (lower level of the Stemm Building) by November 1, 2011. Please call (582-0377) if you need more time or help. Your child cannot celebrate First Reconciliation or First Communion until we have this information. As soon as we record the certificate it will be returned to you.

To obtain a copy of the baptismal certificate, call the church of baptism requesting that a copy of the certificate be mailed to:

If you are sure your child was baptized here at St. Anthony’s it is not necessary to return the baptismal certificate, as those records can be checked. However, this form must still be submitted.

My child was not baptized at St. Anthony and I will drop off/mail the baptismal certificate at the Faith Formation office
My child was not baptized at St Anthony and I have notified the church of baptism to send the certificate to St. Anthony Faith Formation Office.
