Dear Parish members,

This is a time of sadness in our faith community. Your answers to these questions will help us plan how and what we offer to comfort and support each other in the coming months. What you say will be kept within our St. Anthony community; it will not be shared with the Journey in Faith Planning Team or with other churches in our pastorate. Thank you for your honest and thoughtful response.

Please return the form no later than Sunday, April 26 in one of these ways:

·        Place it in a collection basket at one of the April 25-26 weekend masses

·        Drop it off at the parish office, or slip it through the door’s mail slot

·        Submit the online form found on our website under the parish life tab:    www.stanthony-dubuque.org

We are requesting that a Journey in Faith Planning Team representative come to St. Anthony to meet with our community. Your answers to the following two questions will be used in preparation for that meeting.
